Description
The Connections Program is an in‑house health and wellness transition hub for youth 18 – 29 years of age who are moving out of the child welfare system. Participants work closely with a nurse navigator, who conducts a comprehensive wellness assessment to understand each individual’s physical, emotional, and mental health needs. Based on this assessment, the nurse navigator develops a customized care plan that connects youth to primary care providers, mental‑health professionals, addiction services, and, when needed, short‑term psychotherapy and psychological assessments. This program also includes wellness groups, trauma‑focused therapy options, psychiatric evaluations, and ongoing support to help transition out of the child welfare system.
